Appointment Policies:

  • New clients: We ask that you please send your pet’s records in advance to help us prepare for your visit. You may have your records mailed to us at This will ensure you have the most efficient appointment possible – thank you!
  • Deposits: To save your appointment time with the doctor, we will collect a deposit equal to the exam fee. The deposit is forfeited if no-showed or not canceled at least 24 hours prior to the appointment. Please note: If your appointment is on a Monday, you must cancel on Friday.

Our Privacy Policy: Your privacy is protected by law. The personal information below and your pet’s medical records are legal medical documents protected by law against unauthorized release of any and all information. You can be assured your information will not be shared with any person, company or business not immediately associated with this hospital. Your information will be used solely for the purpose of providing veterinary care. Please note: This form must be submitted from an email address already on file with our hospital or in person.

  • Payment Policy: Fees are due at the time services are rendered. There are no payment plans, and we regret that we can no longer accept checks as payment. We recommend Trupanion Pet Insurance to help cover veterinary costs if needed. Care Credit and Scratch Pay are additional payment option.

    43rd Avenue Animal Hospital maintains strict sanitary conditions to prevent the spread of infectious disease. All hospitalized/drop off patients must be properly immunized and free of internal and external parasites. Pets not current on immunizations and/or with internal/external parasites will be vaccinated and/or treated at the customary fees.

  • Your name
